The Child Support Program will need copies of the following documents, if they apply to your case:

  • Divorce Decree, signed by a judge or magistrate
  • Separation Agreement, signed by a judge or magistrate
  • Child and/or Spousal Support Order, signed by a judge or magistrate
  • Divorce Petition (if you are in the process of obtaining a divorce)
  • Complete payment record of all child support paid directly to you if the obligor was ordered to pay you-click here for an Affidavit of Direct Support

Copies of these documents will help speed up the processing of your application.
