Passports

The Jefferson County Clerk's Office has been designated a Passport Acceptance
Facility for the Department of the State of the United States of America.

Requirements:

• Completed DS-11 not signed but filled out
• Proof of Citizenship: Certified Birth Certificate, Naturalization Certificate or Previous Passport
• Identification - with picture ID
• 2 Passport Photos in color 2 inches by 2 inches (we now offer passport photos for a $5.00 per person fee)

Children under the age of 16 need to be  accompanied by both parents.

A young person between the ages of 16 and 18 needs to be
accompanied by at least one parent.

Credit and Debit cards are not accepted.

You will need two checks or money orders for your application.

Appointments are not required, however, in order to process your passport in the
most efficient manner, it would be beneficial if you called prior to coming to our
